1. In the last 24 hours, have you had any COVID-19 symptoms?

2. Are you currently under federal, provincial or local public health orders to isolate or quarantine?

If you answer "YES" to either of these questions, or answer yes on behalf of your child, please do not enter the facility as a spectator, volunteer, visitor, staff member or participant, as applicable.
